Handwriting From the Desk of Bright Teal Notepad

Using an elegant but casual modern handwriting, I've created a graphic reading "From the Desk of" and placed it at the top centre over a teal, blue/green background. Use the template fields to add your personal details, or feel free to delete the bottom field if you only prefer to add your name at the top.
