I would stick with this shape for the best image fit. Customise or delete the back. I am posting a few versions making it easy for you to use as templates. On this, you can change the text to Doctors, EMS (and I am doing another EMS) .... change it all other than the heart. I am doing a couple more with room for the recipient's name; yet, you can make room here or put the initials on the heart or stethoscope.
